Rehabilitation is the process of helping someone get back to a normal, healthy life after something has gone wrong — an injury, an illness, an addiction, or even time spent in prison. Someone who broke their leg does 'physical rehabilitation' (often just called 'rehab') to learn to walk normally again. A person leaving prison may go through 'rehabilitation programs' meant to help them live an honest life and not commit crimes again.
