When you call someone an out-and-out liar, or something an out-and-out disaster, you mean they are that thing all the way through — no half, no mix, no polite . It is a way of stressing the label. You put it before a noun: "an out-and-out cheat," "out-and-out nonsense." It is informal to neutral, still very much alive, with a slightly old, lively flavour. You would not usually say "he is out-and-out" on its own; it wants a noun after it. People also write it with spaces (out and out); it is the same phrase.
out-and-out 用來說某人或某物徹頭徹尾就是那樣，沒有一半，也沒有摻雜，也不會客氣地修飾。它的作用是強調那個標籤，句型是「out-and-out + 名詞」，例如 an out-and-out cheat（十足的騙子）、out-and-out nonsense（完全的胡說八道）。它後面一定要接名詞，通常不會單獨接在 be 動詞後面。
