When something hurts you all of a sudden — you bang a toe, a needle goes in, you burn a finger — the sound that jumps out of your mouth is "ouch." You do not plan it. It is a cry. People say it in real life, often a bit loudly, and writers put it in comics and children's books so you can hear the pain on the page. You can also say it in a small, joking way when the hurt is light, or when you hear a story that sounds painful: "Ouch. That must have hurt." English has other pain sounds too (ow, oww); "ouch" is the one you see most in writing. It is informal. You would not put it in a serious letter.
組成It does not split into pieces. Where it comes from is uncertain, and scholars do not all tell the same story. Some say this cry is American, attested from 1838, and comes from German autsch, perhaps through Pennsylvania German outch. Others say it is just a natural cry, already around in the mid-1600s. The 1933 OED even notes a verb "ouch" in 1654, in a line about someone "Runs Ouching round the mountaine." That old verb is not how people talk now.
