Maniacally means “in a maniacal way”: doing something wildly, as if you have lost control. It is simply maniacal plus -ly, so it goes with a verb — people laugh maniacally, grin maniacally, or work maniacally. Same tone as maniacal: strong and a bit like a film, not a calm medical adverb.
