When someone calls a man limp-wristed, they are saying he does not have the strength or “manly” ways that some people think a man should have. The picture in the insult is a wrist that hangs loose, not held tight. People also use it, in a mean way, to say that a man is gay. It is not a kind word, not a medical word, and not a neutral label. It rests on an old stereotype — the idea that you can “see” a gay man in the way his hand sits. Though actually, a loose wrist tells you nothing of the sort; it only tells you the speaker is using that stereotype. You will still hear it in older films, in cruel jokes, and from people who want to mock a man as weak or as gay. Careful speakers treat it as an insult you should not aim at a real person. Formal writing almost never uses it except to talk about the insult itself.
