Headcase is a rude, informal word for a person who seems wild, not steady in the mind, or hard to deal with — as if something is wrong in the head. You hear it more in British and Irish English than in careful American English. Friends may throw it around as a joke, but it can also be a real insult, and it can hurt people who live with mental illness. It is not a doctor's word, and it is not a kind word. Though actually, case here is the old idea of a person as a “case” — a file, a problem — not a box or a suitcase. You may also see it written as two words, head case.
