Hanky-panky is a joking, slightly naughty way to talk about sexual activity, usually secret or not quite proper — kissing, affairs, “something going on.” People say it to smile and step around a blunter word; it is informal and a bit old-fashioned-playful, more sitcom than clinic. You would not use it in a serious medical or legal talk. Though actually, the word is older than that joke: it also meant trickery or shady dealings, like a magician’s fuss that hides a trick.
