    Something gut-wrenching makes you feel very upset or anxious — as if someone were twisting your stomach. A film scene, a piece of bad news, waiting for a result, a near miss in a match: the feeling sits in the belly, not only in the head. Wrench means to twist and pull hard. People use it for sadness and horror, and also for tension so strong you almost cannot watch. It is informal but very common in reviews and everyday talk. Nothing medical is happening; it is a picture of feeling.

組成gut + wrenching. Gut is the stomach, the place you feel shock; wrenching is twisting hard.
