When something happens gruesomely, it is shocking in a dark way: someone is killed, badly hurt, or the scene is bloody and hard to look at. It is not just “very bad” or “very sad.” A long wait can be unpleasant; a crash that tears a body apart is gruesome. The adverb tells you how something is done or how it looks: “He was gruesomely murdered” means the killing itself is horrible to picture. People sometimes stretch it for things that are only ugly or extreme (“gruesomely ugly shoes”), but that is playful or lazy talk. In careful speech, keep it for death, injury, or horror.
