A foul-up is a mess that happened because someone was careless or stupid — the plan went wrong, the papers got mixed, the wrong button got pressed. It is informal. You might hear it about an office, a trip, or the army: the hotel booking was a complete foul-up. It blames human more than bad luck. A mix-up can be an innocent confusion; a foul-up points a finger, even if kindly. It comes from the verb to foul up, “to spoil something by making a mess of it.” You will also see foulup, or the two words written apart. Though actually, this is not a sports foul (a bad tackle) and not foul weather — here foul is “spoil.” Younger speakers often reach for screw-up or mess-up instead, which land in the same place; foul-up can sound a little last-century, but everyone still understands it.
