A fistful is as much as you can hold in one closed hand: a fistful of coins, a fistful of leaves. It is a rough amount, not a measured one. People stretch it: a fistful of dollars just means “quite a lot of money,” a picture that got extra life from an old film title. The -ful is the same piece as in handful or : “enough to fill this.” A handful is the everyday cousin you will hear more; fistful feels a bit tighter and more physical, as if the hand is really closed around it.
組成fist (the closed hand) + -ful (“as much as it can hold”).
