A fathead is a stupid person. It is a rude name, said in anger or as a joke between people who already know each other. It is not about a head that is fat. The picture in the word is thick in the head — slow to think. It sounds a bit old and a bit comic; younger speakers may reach for newer insults. It is informal. Do not put it in a letter to a teacher. Though actually, it is milder than the worst names: it calls someone foolish, not cruel or evil.
