This is the adverb of extemporaneous: you speak, play, or deal with something without a prepared script, in the moment. Same world as the adjective — classrooms, contests, careful prose. In ordinary talk, people say off the cuff, on the spot, or just without notes. You can plan the shape of what you will say and still speak extemporaneously, as long as you are not reading a finished text. It is a long, formal word; one long word is doing the work of a short phrase.
