Emaciation is the noun for that state: extreme thinness from hunger or illness. A medical note might mention emaciation; a news report might describe the emaciation of people or animals after a famine or a drought. It names the condition, not the person. It is formal — more at home in hospitals and newspapers than at the table, where you would just say someone looked terribly thin. Though actually, even experts often prefer plainer words with families; emaciation turns up when the writing needs a name for the condition itself.
