A boo-boo, in this sense, is a silly mistake — usually a small one, the kind you laugh about after a second, not a disaster. You made a boo-boo if you sent the email to the wrong person, or wrote 15 when you meant 50. It is informal, a little , and often kind: a parent, a nurse, or a friendly boss might use it so the mistake does not feel huge. Grown-ups also use it with a raised , which can sound talking-down if the other person wanted to be taken seriously. You will hear 'make a boo-boo' more than 'commit an error'. Though actually, the same spelling is widely used for a child's little hurt — a knee — and that is a different meaning. Here we are only on the 'oops' sense. People do not use it in careful essays.
組成Probably a doubled baby-talk sound (boo-boo), but the older story is not settled. People have guessed a link to bug-a-boo, to French bobo, to a baby's noise, even to bubo (a plague swelling). None of those is a fact I can stand behind, so the honest answer is: we do not know for sure.
