If a thing is bloodstained, it has a mark or spot of blood on it. You say this about the thing that got the blood — a shirt, a cloth, a knife, a floor — not about the mark as a thing on its own. Though actually, the mark has its own word: a . So a bloodstained shirt is a shirt with on it. The word feels grim and a little serious. News reports, history, and crime stories use it more than everyday chat.
