Blood-and-guts describes a story, film, or game that is full of fighting, injury, and messy violence — you can almost see the blood and the insides. People also stretch it to sport or politics that feel like a brutal fight: “blood-and-guts football.” It is informal. Here guts means the messy inside of the body, not courage (even though guts can mean bravery in other phrases, like “it takes guts”). The phrase was coined in 1937.
