If you answer or act affirmatively, you are saying yes, or showing agreement. A nod can be given affirmatively. She answered affirmatively means she said yes. It is a formal, slightly stiff adverb. In real conversation people just say yes, or she agreed. You will still meet it in reports, in court, and beside the set phrase in the affirmative. Though actually, affirmative itself — an affirmative answer — is the cousin people still use; the -ly form is the one that sounds like paperwork. It does not mean , and it does not mean the plan will work.
