To actualize something is to make a plan or a wish become true — to turn it from an idea into a real thing. It is a formal verb. Teachers, coaches, and business writers like it; friends more often say make it happen, carry it out, or realise (UK) / realize (US) a goal. Actualize your potential is a set self-help shape. British spelling: actualise. Same meaning. You actualize a plan (you make the plan real). You do not actualize a fact in the sense of checking whether it is true — that would be a mix-up with actual meaning real.
